Have you made sure that all printers in the Devices and Printers folder actually exist. Sage 100 has a problem if a printer listed is not actually present.

Typically, you have to create a new form code. Not sure why, but that has always been the case when upgrading to Sage 100 2013 and 2014 from 4.x.
